In 2014, CQ Youth Connect (CQYC) developed a Holistic Health Care Model that empowers Youth, and delivers Holistic Health Care Services and Awareness across Central Queensland. CQYC acknowledges Aboriginal & Torres Strait Islander peoples and we pay our respects to their Elders, past and present who we share this great Country with. Our aim is to empower Youth in making informed Health decisions.

This service offers specialised palliative care consultation and support to prevent unnecessary hospitalisation and suffering with a focus on preparation for care at end of life in a place of residence. When pain or symptoms become difficult to manage you can be supported by a palliative care nurse practitioner.
