About Us
Our team of health professionals, students and supporting staff provide treatment, education, and tools to help you manage chronic disease at home. We aim to reduce the need for future hospital admissions. We communicate with your doctor so they are kept informed of your progress. The clinic is a working partnership with CQ University.

Who can access this service
People who have a complex chronic disease and meet the SACCR eligibility criteria:
-
over 18 years old
-
have a specific chronic disease that fits within the cardiac, respiratory, diabetes, venous compression or complex spinal pain pathway
-
eligible for 2 or more clinicians
-
recent hospital admission (within 3 months) OR multiple hospital re-admissions OR are at high risk of hospital admission
-
willing to participate
How to access this service
You can be referred by your General Practitioner or a hospital staff member. Referrals for complex spinal pain are only available via an orthopedic specialist.
