Albany Community Care

We support seniors and their carers in the Great Southern to enjoy an enhanced quality of life.  Initially founded as a project of the Albany Lions Club in 1986, we pride ourselves in promoting independence and wellbeing.  The only provider in the Great Southern created locals and governed by locals, for our local community.

Albany Community Care (ACC) provides a holistic service to our clients through Home Care Packages, the Commonwealth Home Support Programme, Veterans Home Care and privately.  We deliver home services and have a Social and Wellness Centre onsite.  Some of the services we provide are:

- Case Management: coordinating your care with us and with your other health care specialists

- Personal Care: showers, grooming, medication assistance and general checking in to make sure you are safe and well

- Transport: to assist you to and from appointments and activities to keep you active in your community

- Individual Social Support: at home or out and about, one on one with a Care Provider

- Meal Preparation: we help you prepare your meals or arrange for pre-prepared meals to be delivered to you

- Clinical nursing care: for more complex client needs, including wound care

- Allied Health: in our Wellness Centre or in your home with an Occupational Therapist

- Respite Services: care for the person in need whilst the carer takes a well-earned break

- Home and Garden Maintenance: delivered by our sister company HGM Albany, to ensure your home and your garden are safe, functional and tidy

- Domestic Assistance: helping you to run your household, including cleaning, grocery shopping and bill paying

- Social and Wellness Centre with Men's Shed: open 5 days a week where you can chat with your peers, play games, and go on excursions in a supported environment
